Visit Service Ontario to apply for a marriage, birth or death certificate. Read all instructions carefully. Certificates are vital records and, in most cases, applicants must meet certain conditions and eligibility requirements. The Office of the Registrar General issues and mails certificates.
A marriage certificate is a legal document showing proof of marriage. Please wait approximately 10 to 15 weeks after the date of marriage to apply. This allows time for the Ministry to Government Services to receive and register your Record of Marriage. Applications submitted online will arrive within 15 days.
A birth certificate is a foundation identity document required to apply for other forms of identification, such as a health card or social insurance number. Applications submitted online will arrive within 15 days.
New parents can take advantage of the newborn registration option available through Service Canada.

Please note that death certificates can only be obtained through Service Ontario. Funeral homes can issue a statement of death only.
