The fee per signature is $29.87, plus HST. All proof of life forms are $20.32, plus HST per signature.
The person(s) swearing the affidavit must appear before the commissioner. Any person that is signing the document must be present and provide their government-issued identification that includes a photo and signature (i.e., a driver's licence, provincial photo card or passport).
