A City of Burlington Commissioner of Oaths (also known as a Commissioner for Taking Affidavits) is a person authorized by the Province of Ontario to administer oaths or declarations when you sign an affidavit or statutory declaration.
We commission documents such as: |

- City of Burlington generated documents such as applications for minor variance
- domestic and foreign pension documents (Proof of Life)
- statutory Declaration of Apprenticeship hours
- Canadian citizenship forms
- delayed statement of birth
- name change documents
- affidavit of a lost cheque
- statutory declaration of lost Identification (must include police report number)
- sworn statement for the transfer of a used motor vehicle
- statutory declaration for OSAP - Statement of common law status/supporting children
- statutory declaration of theft of a GST/HST or federal cheque
- affidavit of unregistered vehicle
- statutory declaration for motor vehicle deals compensation fund
- immunization health form
- cohabitation (living in a common law relationship)
- religious or conscientious belief vaccination exemption
- construction completion forms

- wills, living wills, codicils to wills, or estate settlement
- powers of attorney
- divorce, separation, marriage or cohabitation
- court, legal or civil issues
- debt
- tenders
- declaration or claimant and indemnity
- declaration of replacement of shares
- statutory declaration of progress payment holdback security deposits
- declaration of a translator
- age of majority or BYID card applications
- documents not in English or that do not have English descriptors
- certified true copies of documents
- documents requiring a notary public’s signature

Fees
The fee per signature is $30.62, plus HST. All proof of life forms are $20.83, plus HST per signature.
Requirements and identification
The person(s) swearing the affidavit must appear before the commissioner. Any person that is signing the document must be present and provide their government-issued identification that includes a photo and signature (i.e., a driver's licence, provincial photo card or passport).
Privacy of information
