Complete the Specialized Transit application to apply for specialized transit in Burlington, Milton and Oakville. Sections of the form must be completed and signed by your doctor or a health care professional. Review of the application will consider the following criteria:

Review |
| Applications are reviewed by a third-party medical company. As part of the review, you may need to have an in-person assessment. Fully completed applications will be reviewed within 14 calendar days from when they are received.. After the review, you will receive a letter by mail detailing the review. If you are approved, you will receive a welcome package. |
